 1. Please complete these assessments as a basis for our first working appointment at which you will uncover your long-term wellness vision and we will design your three-month goals and one-week goals.

We can also send you a link to this form that'll take a digital signature.

Please complete the documents at least two days before our first appointment to make sure there's enough time for me to review it and really be present and thoughtful for you when we meet.

Just so you know, I have another set of assessment forms that specifically relate to your respiratory condition. It involves putting all your medication prescriptions; recent tests, results and update schedules; and your Action Plan together in one place to complete a comprehensive picture of your status. This may already be available to you with your health-care provider, or you or a family member may need to compile it with my support. For some clients this will be a big project and huge clarifying benefit of the coaching process. It's impossible to know in advance how much of the coach's time this will take to discover. As we uncover in the course of coaching, what is missing and what needs to happen and in what timeframe, we will openly discuss what optional, extra support is needed and bill for that time if necessary.

To build some good momentum, it's important to meet with your coach consistently and reliably. Really give some thought to what is the best day and time during the week for you to be regularly available with the least risk for glitches, other appointments and meetings to potentially intervene. The first coaching appointment is generally longer, so we may have already set something up and that's fine. But think ahead about what would be the best weekly or biweekly time. I can work around regular business hours.
